Older People Mental Health Care Coordinator Job with Visa Sponsorship (COS) – As a Community Care Coordinator, you are not only supporting clients through what can often be the most challenging of times, you are also mentoring them to become the best version of themselves.  Having an open mind and a flexible approach to care is key.  This helps to establish the best possible ways to care for your clients within a recovery-focussed mindset, ensuring that you never lose the motivation that change is achievable for all.  All care that you deliver will be in line with current care pathway frameworks and national guidelines.

You should be caring, flexible, and creative.  Able to thrive under pressure, you will know how to connect with people at all levels.  Your driving passion will be to help others live as independently as possible.

Qualifications

Essential criteria
  • RN (part 13) RMN, Or HCPC registered, OT or Physio or equivalent, Or Social Worker
Desirable criteria
  • Clinical supervisor training

Experience

Essential criteria
  • Minimum of twelve months at Band 5 or equivalent responsibility in mental health
Desirable criteria

Experience in Cognitive / Memory Assessment activity

Important Dates

Experience of working with clients aged 65 years and over who have mental health needs.
